From time to time, it is necessary for you to supply Luen Fung Hang Life Limited ("the Company") with data about yourself(ves),
policyowner(s), claimant(s), beneficiary(ies), life insured(s), and/or other relevant individuals (the "Personal Data") in connection with the provision of products and services to you by the Company or any of its group companies and affiliated companies, the processing of claims under insurance policies issued by the Company, and/or the processing of any and all other requests, enquiries and complaints from you. |

Provision of the Personal Data to the Company by you is voluntary. However,failure to supply the
Personal Data may result in the Company and any of its affiliated companies being unable to
provide products and services to you, process claims under insurance policies issued by the Company, or process any other requests, enquiries, or complaints from you. |

The Company and any of its affiliated companies may collect Personal Data from you for any of the following purposes:

processing and/or approving applications for products and/or services and additions,
alterations, variations, cancellations, renewals, and reinstatements of such products
and/or services which may include, without limitation, insurance, provident fund or scheme, or
other financial products or services; |

offering and providing products and/or services to you from time to time, and administering,
maintaining, managing and operating such products and/or services; |
| (iii)
any claim or investigation, analyzing, processing, assessing, determining or responding of such

preventing and/or detecting crimes, fraud and other dishonest behavior. |

reinsurance and claims investigation companies, relevant insurance industry associations and
federations, and members of such industry associations and federations; |
| (ii)
agents, contractors, business partners, and third party service providers who provide
administrative, telecommunications, computer, marketing, and/orother services to the
Company and/or any of its affiliated companies in connection with the operation of business; |
| (iii)
any person to whom the Company is under an obligation to make disclosure under the requirements of any law binding on the Company or under and for the purposes of any
guidelines issued by regulatory or other authorities with which the Company are expected to

any other person under a duty of confidentiality to the Company which has undertaken to keep

company and/or internet service provider). The Company shall not take any legal or other
responsibility for any of such suspension or termination. |

The information and services are subject to changes without prior notice by the Company at its own
discretion. The Company reserves its final judgment and absolute discretion as to using such
